What is my company (old company) required to send me as far as my 401 K is concerned?

I was searching around on what to do with my 401 K now that I no longer work for a company where I had one and my new company doesn’t offer it. I didn’t even know that my company was suppose to have been sending me updates on my 401 K. I didn’t get updates! How often and what type of statement was it my old company was suppose to be sending me? What should I be asking from them now to be able to roll it over to a Roth or something?
